Enrollment Procedures 2022-2023
The application must be completed on both sides and returned to the school office with the following items:
Transitional Kindergarten: Items A through E
Kindergarten: Items A through E
First and Second Grade: Items A through F
Third through Eighth Grade: Items A through H
A. BIRTH CERTIFICATE – ORIGINAL CERTIFICATE from the country of birth – hospital certificate and copies of the original are NOT acceptable.
B. BAPTISM CERTIFICATE
C. SOCIAL SECURITY CARD
D. IMMUNIZATION RECORDS
E. CUSTODY AGREEMENT (IF APPLICABLE)
F. MOST RECENT REPORT CARD
G. FIRST COMMUNION CERTIFICATE
H. RECONCILIATION CERTIFICATE
Please bring your original items, which will be copied by the office.
1. Please contact the school office at 310-328-5387 to make an appointment for evaluation.
2. Criteria for acceptance:
A. All incoming students must be evaluated. Students’ acceptance is based upon the
evaluation and oral interview.
B. Priority is given to families who have children already enrolled in Nativity Catholic School.
C. Transitional Kindergarten (TK) students must be four (4) years old by September 1st.
D. Kindergarten (KG) students must be five (5) years old by September 1st.
2. Registration will take place at the time you receive your notice of official acceptance.
3. If students are invited to enroll and there is no space available, they will be placed on a waiting list.
Nativity Catholic School, mindful of its mission to be a witness to the love of Christ for all, admits students regardless of race, religion, color, or national and/or ethnic origin to all rights, privileges, programs, and activities generally accorded or made available to students at the school.
